Job Title: Home Health Nurse - RN

Full time, Day/Evening shift. Facility: 100 Spotswood Dr - Lexington CP01 Carilion Medical Center. Requisition Number: R149476 Home Health Nurse- RN (Open).

As part of the health care team, the Home Health Nurse assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team-oriented, collaborative approach to patient care.

• Assesses, plans, implements, and evaluates patient care to achieve optimum outcomes.
• Age specific admission and ongoing assessments are completed, documented and used to develop a plan of care.
• Collaborates with health care team/patient/family to develop, implement, evaluate an age-specific interdisciplinary plan of care and revise as needed.
• Treatments and medications for patients of all ages are accurately administered and documented.
• Identifies patient/family educational needs and incorporates them into the plan of care.
• Performs within clinical, leadership, and education pathways to support evidence-based patient care and performance improvement.
• Exhibits clinical expertise in rapidly changing circumstances.
• Plans strategies to reduce length of stay, turnaround time, and resource consumption to improve outcomes.
• Functions as role model for clinical practice to coworkers, students, and physicians.
• Manages resources, establishes priorities, and modifies environment to meet patient care needs.
• Completes manager/director predetermined projects on time and according to guidelines.
• Uses problem-solving methods to facilitate resolution of patient and personnel situations.
• Takes an active role in orientation of new employees.

Interactions with patients, families, visitors, and health care team. Delegates and oversees delivery of patient care. Maintains confidentiality. Demonstrates knowledge of principles of life span growth and development. May be exposed to infectious diseases, biohazardous and hazardous materials.

Physical requirements:

lifting, standing, bending, pushing, pulling, reaching, walking.

May provide care at other Carilion facilities. Some positions may require call. Supports Culture of Excellence and actively practices Carilion Clinic Mission, Values, and Guiding Principles.

RNs are eligible to participate in the Career Advancement Program (CAP) to attain various levels of advancement.

• Education:
  
  Graduate of school of professional nursing.
• Experience:
  
  One (1) year experience in hospice, home health, oncology, or medical surgical nursing.
• Licensure:
  Currently licensed as Registered Nurse in Virginia; valid V.D.L. and pass Criminal Record Check as per state law.
• Life Support: AHA BLS-HCP required.
• Minimum Qualifications:
  
  Orientation and competency completion; keyboarding skills; meet essential job functions.
