Role PurposeSeeking cardiovascular-experienced registered nurse to join the Advanced Heart Failure Team located at the Cardiovascular Institute in Crystal Spring Tower as a Heart Failure Clinical Coordinator. Supports patients with advanced heart failure/LVADs using advanced nursing processes in decision-making, patient assessment, data analysis, and development and implementation of methods and systems to improve quality and efficiency of patient care for the designated department and population served.
Provides direct and indirect patient care as needed; oversees delivery of patient care; and provides day-to-day management of heart failure programs (Infusion, LVAD, HF remote monitoring) to coordinate patient care as designated by the department. Coordinates the delivery of safe, high-quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements.
Serves as a resource and mentor to staff and team members. Potential nurse call opportunity available.
This position is full time, working Monday-Friday 8a-5p.
Responsibilities- Organizes, formulates, and coordinates a multi-disciplinary approach to the patient population for achievement of optimum outcomes.
- Assesses, implements, and coordinates patient care plans including referrals, admission/appointment criteria, discharge planning with patient education, post-discharge care planning, and ensuring patient/family understanding of the plan of care.
- Problem solves to facilitate effective outcomes and resolves complaints or concerns.
- Plans, implements, and coordinates strategies through evidence-based practices to support quality patient care and improve outcomes (e.g., length of stay, turnaround time, resource use).
- Develops, collects, analyzes, and evaluates data to identify trends; presents findings and makes recommendations for performance and quality improvement, regulatory requirements, and tracking patient outcomes.
- Acts as advocate for patients, protector of patient rights, and provider of psychosocial/cultural support.
- Integrates patient beliefs into the plan of care and provides education; coordinates continuing education for department, staff, patients, and families.
- Serves as liaison between department staff, patients, families, physicians, external vendors/partners to ensure collaborative care.
- Coordinates discharge needs with social work and ancillary services; communicates overall plan of care to direct care nursing staff; updates plan of care as treatment progresses.
- Organizes educational presentations, exhibits, and community events; represents Carilion Clinic at conferences and professional meetings; promotes programs and collaborates with management.
- Performs office functions as defined by the department including scheduling, communications, data collection/analysis, and development of documents and tools for departmental population.
- Participates in department and system performance improvement initiatives; uses analytical thinking to evaluate care coordination and interventions on quality outcomes.
- Performs concurrent medical record reviews and analyzes data in accordance with guidelines; supports quality improvement projects.
- Performs in accordance with departmental policies; may have 24x7 responsibility and travel for role-specific education as needed.
Education: Graduate of an accredited School of Nursing required. Bachelor’s degree preferred.
Experience: Three years of RN experience required. At least 2 years in relevant specialty or department (e.g., hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery) preferred.
