- Manage a patient caseload, completing admissions, assessments, and ongoing visits
- Develop, implement, and update individualized Plans of Care with the Interdisciplinary Team
- Provide skilled nursing care including symptom management, wound care, IV therapy, and assessments
- Educate patients and families on comfort care, medications, and daily caregiving needs
- Document accurately and timely while ensuring regulatory compliance
- Participate in IDT meetings, on-call rotation, and quality improvement activities
- Graduate of an accredited nursing program with an active Virginia RN license
- Minimum of 2 years nursing experience (hospice/home health or acute care preferred)
- Comfort with EMR and technology
- Reliable transportation with valid driver’s license and insurance

Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM), Serving patients primarily in the Franklin County area.
Benefits: 21 days of PTO in your first year, $.65 per mile mileage reimbursement, company paid life insurance and long-term disability insurance, medical through an ICHRA model offering multiple plan choices, and 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)
