Responsibilities
:
Coordinate medical and behavioral health care for individuals receiving substance use treatment services. Ensure continuity of care, facilitate communication among providers, and support clients in accessing necessary medical, psychiatric, and social resources. Requires knowledge of ASAM criteria, DMAS regulations, and integrated care principles.
Education and Experience Required
: RN, LPN, CNA, CMA, or other medical training; experience with SUD and mental health preferred.
Job Details
:
Expected to work Monday through Thursday (2‑4 hours per day). Hybrid potential.
