Hospice Social Worker / MSW (Full‑Time)

Roanoke, VA (Roanoke County, Boteourt, Buchanan, Vinton, Salem, Franklin County, Bedford, Moneta)

Base pay range: $48,000.00 /yr – $55,000.00 /yr

• Provide social work services in accordance with the physician’s plan of treatment.
• Work with the interdisciplinary team directly or through coordination of team members to provide appropriate assessment and support patients and families.
• Provide assessment for normal and complicated grief and provide bereavement counseling as appropriate.
• Serve as a liaison with other community resources.

• Master’s or Bachelor’s level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one‑year Hospice experience preferred.
• Valid Driver’s License.
