Counselor On-Call - Hospice

At Good Samaritan Hospice, we consider it a sacred honor to walk with individuals and families through the end‑of‑life and grief journey. We are seeking a compassionate, grounded, and skilled Social Worker to join our Roanoke team as a Counselor On‑Call. • Provide on‑call psychosocial support to hospice patients, caregivers, and families by phone and in person.
• Respond promptly during assigned on‑call shifts and review reports, emails, and messages at shift start.
• Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
• Support admissions, changes in level of care, and end‑of‑life needs in collaboration with the after‑hours care team.
• Complete accurate, timely documentation and shift reports per agency standards.

• Master’s degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
• At least three years of social work or counseling experience; hospice or end‑of‑life care preferred.
• Strong skills in crisis intervention, grief support, case management, and critical thinking.
• Excellent communication, collaboration, and ability to work independently.
• Compassionate, emotionally mature, and mission‑driven with a heart for end‑of‑life care.

Schedule:

PRN (as‑needed) position. Team members are required to pick up a minimum of 15‑20 shifts per month.

Available shift options include:

• Weekdays: 5:00 PM – 8:30 AM
• Flexibility to work a mix of weekday and weekend shifts is expected.
