We are seeking a compassionate, grounded, and skilled Social Worker to join our Roanoke team as a Counselor On‑Call. We are searching for someone who can offer hope, healing, and connection during some of life’s most profound moments.

- Provide on‑call psychosocial support to hospice patients, caregivers, and families by phone and in person.
- Respond promptly during assigned on‑call shifts and review reports, emails, and messages at shift start.
- Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
- Support admissions, changes in level of care, and end‑of‑life needs in collaboration with the after‑hours care team.
- Complete accurate, timely documentation and shift reports per agency standards.
- Master’s degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
- At least three years of social work or counseling experience; hospice or end‑of‑life care preferred.
- Strong skills in crisis intervention, grief support, case management, and critical thinking.
- Excellent communication, collaboration, and ability to work independently.
- Compassionate, emotionally mature, and mission‑driven with a heart for end‑of‑life care.
Schedule:
PRN (as‑needed) position. Team members are required to pick up a minimum of 15‑20 shifts per month.
Available shift options include:
- Weekdays: 5:00 PM – 8:30 AM
- Flexibility to work a mix of weekday and weekend shifts is expected.
