Position Summary
The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family. The Care Manager facilitates the plan of care with the objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. The Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day-to-day workflow, education, process improvements and patient satisfaction).
The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care for every patient.
- Manage, coordinate, and facilitate patient care progression throughout the continuum.
- Complete psychosocial assessments using departmental procedures and within the required time frames.
- Work collaboratively and maintain active communication with providers, nursing and other members of the health care team to effect timely, appropriate patient management.
- Utilize critical thinking, assessment findings and clinical skills to lead interdisciplinary team in plan of care development and implementation.
- Identify delays or alterations in the plan of care and address/resolves system (internal and external) problems impeding diagnostic or treatment process to ensure delivery of care in the most appropriate setting.
- Make the necessary arrangements to facilitate and coordinate support services to significantly improve outcomes, through safe, sustainable plans of care and appropriate follow up.
Education:
Bachelors of Science in Nursing or higher, or Bachelors of Science in Social Work or higher is required.
If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.
Experience:
3-5 years of Medical Case Management.
Clinical Coach Certification required.
Pay Range$65,520.00 - $95,501.12
