The Staff Nurse in Home Care Services provides professional nursing care to patients of all ages in their homes, ensuring safety, comfort, and quality of care. The nurse assesses patient needs, implements individualized care plans, monitors patient progress, and coordinates with the multidisciplinary team. The role supports adherence to standards of professional nursing practice, CMC policies, CBAHI and JCI standards, and promotes patient-centered care in a home environment.
FunctionalResponsibilities:
- Ensures through his actions, either directly or indirectly, that a superior quality of medical treatment, compassion and understanding is given to every patient served at the CMC Hospitals, Clinics, and Homes.
- Develop and implement individualized nursing care plans in collaboration with the patient, family, and home care team.
- Administer medications, treatments, and procedures according to physician orders and nursing protocols.
- Monitor patient progress, record vital signs, and report changes in condition promptly.
- Provide wound care, injections, IV therapy, and other nursing procedures as required.
- Coordinate care with physicians, therapists, social workers, and other healthcare providers.
- Ensure proper documentation of all nursing care activities in patient records.
- Maintain infection control and safety standards during home visits.
- Participate in multidisciplinary case discussions and continuous quality improvement initiatives.
- Respond to patient emergencies and provide immediate care as needed.
- Educate patients and families on disease management, medications, nutrition, and preventive health practices.
- Responsible to communicate, educate patients /families as per their needs and to implement their whole role regarding patient / family education process as mention in patient & family education policy and procedure including documentation in patient medical record.
- Involves the patient/families in plan of care and respects patients/families regarding their goals and choices of patient care.
- Provides health promotion and health teaching thru methods appropriate to a patients’ developmental level, learning needs, readiness and ability to learn, language preference, culture and situations.
- Allots time necessary to assess, plan implement and evaluate patient education provided to patients/families with appropriate documentation.
- Abides by the Policies and Procedures of the Department of Home Care Services and By-Laws and Rules and Regulations of Care Hospitals.
- Perform other applicable tasks and duties assigned.
A minimum of:
Education:
Required- Bachelor of Science in Nursing/ Diploma in Nursing.
- Current Licensure (RN) with a nursing governing body from country of origin.
- Professional Nursing License from Saudi Council for Health Specialties.
- Valid BCLS, ACLS certificates from AHA or SHA.
- Minimum of two (2) years of clinical nursing experience;
- 1 year in home care, community health, or outpatient services preferred.
