Family Resource Coordinator​/Assessment Worker

• Review hospital or clinic records and interview families prenatally or immediately after birth to determine families eligible for services through the Healthy Families/Healthy Start Program.
• Conduct comprehensive assessments within the hospital using standardized protocols and instruments and design individualized interventions for families to improve pregnancy and childhood health outcomes.
• Perform other duties as assigned.

• Location:
  
  St Josephs Hospital South – Riverview, Florida and St Joseph's Women’s Hospital – Tampa, Florida
• Status:
  Part time, 28 hours per week
• Schedule:
  
  Saturday & Sunday 7:00 AM – 5:30 PM at St Joseph's Hospital South;
  Additional weekday (flexible) - 8 hour shift at St Joseph's Women’s Hospital
• Weekend Requirement:
  Every
• On Call:
  No

• Bachelor’s Degree in a related field with 2 years’ experience working with women, children or Family Services OR
• High School Diploma with 3 years’ experience with Healthy Families program
