• Competitive compensation of $145,399- $174,478 with quarterly bonus
• Generous time off – weekends off, 17 days PTO, 6 sick days, and 12 paid holidays

• Regular hours of 8am-5pm, Monday through Friday, including home visits

• Licensed Nurse Practitioner or Physician Assistant
• Minimum two years experience working with the elderly
• Prescriptive authority, DEA registration, and eligible to serve Medicare/Medi-Cal participants
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Salary/Wage base range for this role is $145,399 - $174,478/year + Bonus.
