Responsibilities include providing direct patient care, administering medications, monitoring patient health status, and communicating with physicians and families about care plans. The nurse will also ensure adherence to care protocols, maintain accurate medical records, and support patients overall well-being by addressing their physical and emotional needs. Collaboration and teamwork will be key in delivering the highest standards of home-based care.
EducationEducation: Graduate of an accredited school of vocational/practical nursing.
CredentialsCredentials: Current license within the State or valid compact. Current Basic Life Support license (BLS).
ExperienceExperience: 1+ years’ experience as an LVN/LPN in Hospice, Palliative, or Home health, or similar field is preferred.
Responsibilities- Care for ill, injured, or convalescing patients
- Provide basic patient care and treatment
- Collaborate with registered nurses to administer prescribed medications
- Sterilize and prepare medical tools and equipment
- Educate and update patients on medical treatments
- Previous experience in nursing or other medical fields
- Familiarity with medical tools and equipment
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
Compensation: $28 - $35/hr + Mileage
Employment Type: Full Time
- Swing Shift (4/2 schedule, 4:30p–12:30a) – Riverside / San Bernardino area
- AM Shift (Mon–Fri, 8:00a–4:30p) – Temecula to Lake Elsinore
- QA AM Shift (Mon–Fri, 8:00a–5:00p) – Riverside office (in office 2–3x/week)
- Swing Shift (4/2 schedule, 4:30p–12:30a) – Low Desert (Palm Springs / Indio)
- NOC Shift (4/2 schedule, 12:00a–8:30a) – High Desert (Palliative/Hospice)
- AM Shift (Mon–Fri, 8:00a–4:30p) – Low Desert (Palm Springs / Indio)
