Overview
Nurse Practitioners are registered nurses with specialized advanced education and clinical competencies. The Nurse Practitioner works in collaboration with the Medical Director and the Care team to provide services to patients and families in a variety of patient care settings. The Nurse Practitioner will perform initial evaluations for patients coming onto Palliative or Complex Care services and provide follow up visits for patients, as necessary.
The Nurse Practitioner will perform clinical procedures for patients as privileged to do, and will document all assessments, evaluations, consultations, and progress notes.

- Education:
Graduated from an accredited School of Nursing Program. Graduated from a Nurse Practitioner Program approved by the Board of Registered Nursing with specialty as a Family Nurse Practitioner or Adult Nurse Practitioner. - Credentials:
Must have a current Registered Nurse license, a current unrestricted Nurse Practitioner license issued by the State, a current Drug Enforcement Agency (DEA) Certificate. Palliative Care Certification preferred. Current Basic Life Support license (BLS). - Experience:
Minimum of 1 year experience in Hospice, home health, palliative, or similar is preferred. - Salary:
Expected Salary Range: $150,001 – $190,000 annually. Compensation is determined by level, experience, and internal equity. - Core Competencies:
Possess critical thinking skills, in-depth knowledge of clinical disease entities common to Hospice, Home Health, Complex Care management and Palliative Care including the pathophysiology, disease progression, pharmacological and non-pharmacological treatment modalities for Palliative Care, in-depth knowledge of the philosophy, the Medicare benefit, and Conditions of Participation. - Skills:
Excellent observation, verbal and written communication skill. - Other:
Valid driver’s license and auto insurance.
