Base pay range

$30.00/hr - $34.00/hr

Position and reporting

Position: Licensed Vocational Nurse/Licensed Psychiatric Technician

FLSA Status: Non-exempt (hourly)

Reports To: Registered Nurse/QIDP/Executive Director

Normal shifts: All shifts available, including weekends

Summary

The LVN/LPT staff will work with individuals with developmental disabilities in a community-based, residential facility. They will support the medical and health functions of each client in the facility(s) assigned and perform all duties pursuant to the job responsibilities. Successful completion of LVN/LPT programs with CA issued license.

Job Description

An LVN/LPT shall perform duties as directed by the posted duty list and from Facility RN/Manager/QIDP/Executive Director. All duties are expected to be completed successfully to meet performance requirements.

  • Responsible and competent in following the active treatment plan and implementing goals and objectives from the ISP (Individual Service Plan).
  • Assist the Facility RN, Physician and other health professionals including but not limited to keeping close contact and frequent communications in updating clients’ status as needed.
  • Ensure that the DCS are capable and competent in completing nursing/health related tasks appropriately and accordingly.
  • Provide direct care duties to all clients
  • Know the client program plans and activity schedule.
  • Must learn and understand Clients’ Rights.
  • Responsible for documentation in all required areas within the medical program book for follow through of treatment and data collection.
  • Report any unusual occurrences.
  • Responsible as a Mandated Reporter related to the abuse policy.
  • Responsible for house cleaning duties including but not limited to cooking
  • Meal are considered “on duty”. Employees are not relieved from their duties to have their meals and agree to an on‑the‑job paid meal period.
  • Adhere to the Company’s Standard of Conduct and Policies and Procedures according to local, state and federal regulations.
  • Follow additional directives from RN/QIDP/Executive Director when applicable.

You are also required to attend in-service training 3 hours per month and other staff meetings to gain further knowledge of programs and operations. LVN are to ensure the safety for clients and homes must be maintained at all times, safety checks are routinely done. As well as oversee the maintenance and care of all medical equipment and supplies, is required.

Essential

Duties
  • At the beginning of the shift, licensed staff will read the communication book to ensure continuity of care and understanding of all clients’ conditions.
  • Licensed staff to ensure all medications are administered by checking bubble packs and reviewing the Medication Administration Records (MAR).
  • Report any discrepancy on the bubble pack or MAR to the Primary Care Physician, RN and QIDP.
  • Administer medications on time. Check parameters before giving medications, e.g., check blood pressure prior to hypertension medication.
  • Make medical appointments as needed and document in the “Medical Appointment” folder.
  • Take clients to medical appointments and follow up on changes of orders and recommendations from medical appointments.
  • Ensure daily body checks for all clients; document findings and follow Staff Report Procedure as needed.
  • Monitor staff during medication passes and mealtimes as indicated.
  • Assist staff with ADLs and implementation of plans and client interactions.
  • Daily charting on each client; document any acute health issue/problem daily until resolved.
  • Check medication cabinet; ensure external and internal medications are stored separately and check for expired medications and supplies.
  • Order refill medications from the pharmacy when medications are low.
  • Communicate with RN for any change in client condition or any medical/health matter.
  • Maintain charting daily and as needed.
