Join the team at Bridge Home Health & Hospice as a Registered Nurse Case Manager (RNCM) in Home Health. Base pay range: $60.00/hr - $70.00/hr actual pay based on skills and experience. Req #403.

Job Description Summary

The Registered Nurse Case Manager plans, organizes and directs care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions/Responsibilities
  • Patient Care:
    Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process as defined in the state Nurse Practice Act.
  • Assesses and evaluates patient status by writing and initiating the plan of care; regularly re-evaluates patient and family/caregiver needs; revises the plan of care as necessary.
  • Initiates the plan of care and makes revisions as patient status changes; uses health assessment data to determine nursing diagnosis.
  • Develops a care plan with goals based on nursing diagnosis; includes patient and family in planning; initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
  • Counsels patient and family in meeting nursing and related needs; provides health care instructions as appropriate; assists the patient with activities of daily living and supports self-sufficiency and comfort care.
  • Maintains current documentation on EMR per policies and laws; coordinates patient care for an assigned caseload; may coordinate intake and admissions.
  • Communication:
    Completes, maintains, and submits accurate clinical notes; communicates with physician regarding needs and change in condition; coordinates with community health personnel; educates patient and family on self-care; ensures a safe environment; attends interdisciplinary group meetings.
  • Additional Duties:
    Participates in on-call duties; ensures arrangements for equipment and services; supervises ancillary personnel; pursues ongoing professional development; adheres to attendance and reliability guidelines.

Job Qualifications
  • Graduate of a National League for Nursing accredited school of nursing.
  • Current licensure in state of practice and CPR certification.
  • Valid California Driver’s License with proof of auto insurance.

All States:
Registered nurses shall have a minimum of one year of experience as a professional nurse within the last three years or have a baccalaureate degree in nursing from an NLN-accredited program and a current RN license. Minimum two years’ experience is preferred, with at least one in public health, home care, or hospice nursing. Must be a licensed driver with insured vehicle.

Ability to work with minimal supervision and strong communication skills. Physical ability to perform nursing skills and lift/move patients and equipment as required.

Physical Requirements
  • Ability to sit, stand, drive, and perform tasks as described, including lifting up to 50 pounds and frequent walking or standing as required by the role.
