Consumer Services Coordinator (Case Management) – Riverside School Age Central

Under the direct supervision of the Program Manager, the Consumer Services Coordinator (CSC) is responsible for coordinating services and supports available to persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. The position has no supervisory responsibility.

Essential Duties and Responsibilities
  • Provide case coordination services for consumers.
  • Utilize Person‑Centered Planning strategies to create and execute Individual Program Plans, generate progress reports, and conduct annual reviews.
  • Evaluate consumer progress on an ongoing basis; create and implement IPP addendums as needed.
  • Advocate for consumers and families; attend IEP, ITP, IHC meetings, and prepare paperwork.
  • Identify and utilize generic resources for consumers and families.
  • Deliver crisis intervention and document activities.
  • Complete incident reports promptly; inform representatives of other agencies.
  • Coordinate consumer assessments (medical, psychological, developmental, educational, vocational, OT/PT).
  • Handle placement and liaison activities as required.
  • Maintain Medicaid waiver standards and documentation.
  • Arrange placements in residential facilities, day programs, or other training programs as approved by the Interdisciplinary Team.
  • Conduct facility audits (ICRC 513 form) when required and identify facility problems.
  • Identify program situations requiring Quality Assurance referral and refer accordingly.
  • Inform manager of caseload status and difficult case situations.
  • Participate in case conferences and interagency meetings.
  • Provide coverage for absences of other CSCs.
  • Serve as a mentor for new employees as directed.
  • Complete IRC orientation and training sessions.
  • Visite Level 2–4 residential facilities and day programs as assigned.
  • Participate annually in at least one quality assurance evaluation/audit.
  • Meet administrative requirements, purchase of service forms, document Target Case Management notes achieving a minimum of 400 units per month, and complete at least 95% of case‑related paperwork on time.
  • Facilitate purchase of services identified in the IPP.
  • Prepare for compliance and eligibility reviews.
  • Remain abreast of relevant knowledge base; attend mandatory and authorized training, earning continuing education units.
  • Contribute to team harmony, answer questions, share expertise.
  • Adapt flexibility to changing priorities.
  • Perform in a hybrid environment (office, remote, field).
  • Organize travel efficiently.
  • If bilingual, utilize language skills.
  • Use office equipment appropriately; keep workspace neat; follow safety rules.
  • Utilize IRC IT systems and follow security protocols.
  • Maintain consumer rights and dignity.
  • Comply with IRC personnel policies and procedures.
  • Perform additional work as assigned.
Minimum

Position Requirements
  • Bachelor’s degree from an accredited institution and at least one year of case‑management experience in intellectual/developmental disabilities, social work, special education, or a related field; or a Master’s degree in one of these areas.
  • Bilingual preferred but not required.
  • Strong communication, teamwork, and relationship building skills.
  • Good attendance and punctuality.
  • Ability to follow oral and written directions.
  • Valid California driver’s license, automobile liability insurance, or ability to provide independent transportation; safe driving record required.
Other Information
  • Case Management Trainee position available for candidates meeting educational but not experience requirements.
