Preschool Services Coordinator (Case Management) – Riverside Preschool West

Under the direct supervision of the Program Manager, the Preschool Services Coordinator is responsible for coordinating the services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. The coordinator will perform related work as assigned and has no supervisory responsibilities.

Sign-On Bonus
  • $250 sign-on bonus after completing 90 days of employment at IRC with work performance in good standing.
  • $500 sign-on bonus after completing 6 months of employment at IRC with work performance in good standing.
Essential Duties and Responsibilities
  • Assume daily Consumer Services Coordinator responsibilities under the direction of the Program Manager.
  • Evaluate consumer progress and plan as identified in Individual Program Plan (IPP) on an ongoing basis.
  • Maintain contact with consumers and their families as required.
  • Provide advocacy services and attend all relevant meetings (i.e., IEP).
  • Seek out and effectively utilize generic resources on behalf of consumers; provide information and referral services.
  • Under the direction of the Program Manager do individual or family crisis intervention and appropriately document activities.
  • Complete special incident reports in a timely manner.
  • Coordinate consumer assessment including nursing, medical, psychological/developmental, education or vocational and other areas as needed; do follow-up as needed.
  • Provide complete case coordination coverage to any consumer whose Consumer Services Coordinator is absent.
  • Coordinate, implement and update the CDER, Annual Review, and IPP process.
  • Act as the program’s/regional center’s liaison to at least two community agencies, parent groups or programs.
  • Provide written reports of activities to the Program Manager at least annually.
  • Arrange for placement of consumers in residential facilities and/or other programs as clinically indicated and approved by the Interdisciplinary Team.
  • Provide case coordination services to consumers in all levels of community care facilities.
  • Represent the regional center at case conferences and interagency meetings.
  • Keep Program Manager informed of the status of the caseload and community liaison activities.
  • Act as the program sponsor for new employees by providing training in agency philosophy, mission, values, policy and procedures.
  • Lead or participate in at least one Quality Assurance evaluation or audit of a community care facility.
  • Complete all assignments arising out of the agency’s Performance Contract.
  • Daily, complete administrative requirements, purchase of service forms and consumer file record documentation in accord with agency policies.
  • Maintain an adequate skill level in interpersonal and community relationships.
  • Comply with the Agency's Injury and Illness Prevention Plan.
  • Answer questions, share expertise, and generally contribute to the efficiency and training of the team.
  • Use office equipment appropriately and keep work area neat and orderly.
  • Handle change well and be flexible and adaptable in dealing with interruptions and new assignments.
  • Maintain good attendance and punctuality.
  • Utilize agency’s IT systems as assigned, maintaining security and following agency protocols.
  • Seek to stay current and expand relevant knowledge base.
  • Organize travel efficiently and maintain a safe driving record.
  • Utilize bilingual skills in all aspects of the job as required.
Minimum

Position Requirements
  • Master’s Degree in social work, psychology, public health nursing, teaching special education, early childhood development, or related field from an accredited college or university.
  • OR BA or BS degree from an accredited college or university and one year of experience including case management in intellectual/developmental disabilities, social work, special education, early childhood development or related field.
  • Bilingual preferred.
