We are seeking a Full‑Time Hospice RN Case Manager to provide care in Fond du Lac and Dodge Counties

• Makes the initial nursing evaluation to help determine eligibility for hospice services within twenty‑four (24) hours of referral.
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re‑assesses as needed, no less than every fifteen (15) days.
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
• Collaborates with the patient/family, attending physician, and other members of the IDG in providing patient and family care daily.

• Two years of clinical experience.

• Current Wisconsin RN licensure and two years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

  Full‑Time, 1st shift Monday‑Friday. Minimal on‑call required.
• Compensation: Negotiable based on experience.
