Community Staff Nurse – Dorset Health Care University NHS Foundation Trust

An opportunity has arisen for a dynamic Registered Nurse to join the District Nursing Service covering East Dorset; full-time hours are available within our Crane Valley team.

The role of the Community Staff Nurse has evolved to support complex patients in their home environments. Skills required include complex wound management, palliative care and end of life care, medicines management, IV drug administration, and a high standard of holistic assessment and care planning.

Training is available within the Trust for the successful applicant. The team works closely with wider multi-disciplinary teams including local Hospices, Complex Care Nurses, Health and Social Care Coordinators, Specialist Nurses and Therapist colleagues.

The team is supported by Intermediate Care Service and utilises our community hospitals for patients’ rehabilitation.

Main duties of the job

Care of patients at home requiring nursing care, including diabetes management, complex wound care (including compression bandaging & dopplers), IV management, end of life care, injections and pressure area care, among others.

Provide a high quality person-centred approach to care delivery that always considers people’s safety, privacy and dignity.

Undertake assessment of patients/service users’ care needs and develop programmes of care as part of the wider multi-disciplinary care team.

Provide advice to patients/service users and carers regarding their care.

Plan, implement and review health improvement programmes in a range of settings.

Provide supervision, mentorship, and clinical support to students and junior staff in the team.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of requiring a new check.
