**** Position Purpose summary:
** Rotary Hospice House is a community that honors life, values humility, and shares hope. As an LPN, you treat all residents, volunteers and staff members with respect and courtesy. You regard the dignity and best interests of each resident, recognizing that they have a right and an obligation to take responsibility for personal actions and choices. You are expected to fulfill your LPN obligations and responsibilities with integrity, and perform every duty in a conscientious, diligent, and efficient manner.

* KEY RESPONSIBILITIES:

*** Assumes responsibility for the delivery of care for each resident, ensuring that all personal and nursing care is provided in accordance with the mission of The Salvation Army, agency, regulatory and established ethical guidelines and requirements.
* Monitors the treatment regime and health status including physical, emotional, spiritual and psychological needs to ensure the safety and well-being of the residents.
* Apply critical thinking skills to assess the resident’s clinical status.
* Demonstrates medication knowledge for the purpose of assessment and administration as per physician orders and Medication Standards according to the BCCNM.
* Performs nursing care to residents including administering prescribed medication and treatments, and observing, recording and reporting symptoms and changes in resident condition; offers compassionate care by understanding the goals and values of residents.
* Communicate clearly and respectfully with all members of the hospice team, with residents and their family members and can effectively and patiently collaborate toward arriving at the best practice and best care of residents.
* Carries out established procedures to offer care for residents, is aware of the upkeep of the facility’s equipment (such as fridge temperatures, battery operated devices, emergency equipment, OHL and oxygen concentrators) and uses supplies according to plan of care.
* Provides caring environment to residents and families including facilitating participation of residents and family in care plans.
* Maintains pertinent records and ensures all documentation is accurate and complete. Maintains strict confidentiality of information about residents and families.  Documents in electronic health record and in the E-MAR all medication reconciliations.
* Participates in professional development opportunities such as in-services to maintain current knowledge and practice.
* Maintains relationships with all external health agencies, professionals and the community.
* Performs other related duties as required.
** MANAGERIAL/ TECHNICAL LEADERSHIP RESPONSIBILITY:
** The LPN is supervised by the Director of Care and the Executive Director of the Rotary Hospice House.

* WORKING CONDITIONS:

*** The LPN will be working at a Hospice in Palliative care environment.
* The LPN role requires a frequent amount of walking, as well as physical ability to provide direct care.
* Lifting maximum of 50lbs when needed
* Emotional and sensitive environment with frequent exposure to death and grieve as residents pass on.
* Effectively seeking help from co-workers when 2 people are required to safely deliver care.
