Carebridge LPN/LVN

Virtual: This role allows associates to work virtually full‑time with required in‑person training sessions. Candidates must be within a reasonable commuting distance, unless accommodation is granted by law.

Work Shift: Full time (40 hours/week) position, 8 am‑5 pm local time with holidays and weekend rotation.

• Review patient medical records, interview patients and record vital signs.
• Provide nursing interventions and coaching in accordance with the scope of practice and standing orders.
• Facilitate patient transfers to an alternate level of care.
• Document using standard templates.
• Engage with patients on the phone up to 80% of the shift.
• Work assigned weekends per standard team rotation.
• Maintain reliable internet and high level of customer service.

• Licensed LPN or LVN with minimum 2 years of experience (or equivalent education/experience).
• Current LPN/LVN license in the applicable state.
• For Carelon Health business unit, satisfactory completion of a Tuberculosis test is required.

• Bi‑lingual (English/Spanish) preferred.
• Experience with telephonic triage preferred.
• Acute‑care experience (e.g., urgent care or ER) preferred.

Salary: $24.99–$31.25 per hour (New Jersey and Washington State). Elevance Health offers a comprehensive benefits package including incentive and recognition programs, equity stock purchase, and 401(k) contribution.
