We are seeking a compassionate and dependable Hospice LPN to join our interdisciplinary team. This role is ideal for a nurse who values relationship-based care and wants to make a meaningful difference in patients’ lives.

• Under the supervision of a Registered Nurse, the Hospice LPN provides skilled nursing care to patients in home and facility settings in accordance with the individualized Plan of Care.

• Provide skilled nursing visits as assigned
• Perform focused assessments and report findings to RN Case Manager
• Administer medications and treatments as ordered
• Monitor pain and symptom management effectiveness
• Provide wound care per physician orders
• Reinforce patient and family education on disease process and care plan
• Assist with coordination of care alongside the interdisciplinary team
• Participate in IDG meetings as appropriate
• Complete timely and accurate documentation in EMR

• Active, unrestricted Virginia LPN license
• Hospice or home health experience preferred
• Strong assessment and communication skills
• Knowledge of end-of-life care principles
• Current CPR certification
• Valid driver’s license and reliable transportation
• Ability to travel throughout service area (mileage reimbursed)
