We are seeking an experienced and compassionate Hospice RN Case Manager to join our interdisciplinary team. This role is ideal for a nurse who values autonomy, relationship-based care, and making a profound impact on patients and families.
✨ What You’ll DoAs an RN Case Manager, you will oversee and coordinate the total plan of care for a caseload of hospice patients in home and facility settings.
🩺 Clinical Leadership- Perform comprehensive patient assessments
- Develop and update individualized Plans of Care
- Manage symptoms and provide end-of-life education
- Coordinate care with physicians and hospice Medical Director
- Supervise Hospice Aides / Home Maker Aides
- Ensure compliance with Medicare Conditions of Participation
- Participate in IDG meetings
- Collaborate with social workers, chaplains, aides, and therapy services
- Provide education and emotional support to patients and families
- Facilitate transitions in care levels (Routine, GIP, Respite, Continuous)
- Ensure timely documentation and regulatory compliance
- Complete timely and accurate EMR documentation
- Maintain audit-ready clinical records
- Follow agency policies and state/federal hospice regulations
- Active, unrestricted Virginia RN license
- 1–2 years of hospice experience preferred
- Strong assessment and symptom management skills
- Knowledge of Medicare hospice regulations
- Current CPR certification
- Valid driver’s license and reliable transportation
- Excellent communication and time management skills
- Competitive salary (Richmond market range typically $75,000–$90,000)
