Nurse Practitioner – Part Time, Richmond, VA

This role involves delivering comprehensive primary and acute medical care to residents in post‑acute and long‑term care facilities. The successful candidate will collaborate with multidisciplinary teams to provide safe, person‑centered care, manage medications, support transitions of care, and contribute to continuous quality improvement.

- Perform comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
- Provide diagnosis, treatment, medication management and follow‑up care in line with current evidence‑based practice
- Respond to acute changes in condition, coordinate timely interventions and facilitate transfers of care when appropriate
- Collaborate with nursing staff, physicians, therapists, social workers and facility leadership to promote holistic, person‑centered care
- Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
- Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
- Support medication safety, antimicrobial and antipsychotic stewardship, and quality improvement initiatives
- Provide clinical consultation and education to facility staff and medical colleagues on complex clinical issues and best practices
- Maintain professional standards of practice, including infection control, documentation and regulatory compliance
- Master’s degree in nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
- Active and unrestricted Virginia APRN and nurse practitioner licensure (or eligibility for licensure)
- Prescriptive authority as required by Virginia state regulations
- Certification in a relevant specialty (e.g., family, adult‑gerontology) preferred
- Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
- Proficiency with Electronic Medical Record systems and clinical documentation
- Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
- Commitment to patient‑centered care, professional integrity and ethical practice
