• Perform visits and coordinate client care, acting as the primary point of contact for all disciplines involved.
• Oversee visit frequency for each episode of care.
• Consult with physicians and office staff, providing detailed patient care updates.
• Accurately collect OASIS data, maintain current medication profiles, and ensure lab values are timely reported to physicians.
• Attend weekly case conferences and monthly case manager meetings.

• Licensed as a Registered Nurse (RN) in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrated knowledge and skill in current nursing practice.
• Valid state driver’s license and automobile liability insurance.
