Overview
The Transplant Coordinator delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This role safely administers prescribed medications, identifying incompatibilities and/or interactions. It demonstrates a high level of expertise in the assessment, coordination and care of complex patients, families, or communities to address actual or potential health problems;
prevention of illness and injury; maintenance of wellness; and provision of comfort.
The Transplant Coordinator demonstrates strong clinical experience in patient care and a high level of expertise in the assessment, diagnosis, and treatment of complex patients and their responses to health problems. The position assists the health care team with prevention of illness and injury, maintenance of wellness, and provision of comfort, and provides direction, education, and leadership.
ResponsibilitiesThe following responsibilities apply to the role, aligning with documented orders and standard practice guidelines:
- Follow documented physician/licensed independent provider orders, within the Nurse Practice Act and established standards, policies and practice guidelines.
- Organize and plan work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources.
- Handle day-to-day operations independently with minimal assistance.
- Seek advice and guidance from the Manager or Director on an as-needed basis and when there is a major impact on activities outside the unit.
- Perform actions within the scope of practice, including leadership of staff and daily operational functions.
- Current RN licensure in Virginia or eligible
- Current RN licensure in Virginia
- AHA BLS HCP Certification or equivalent
- Minimum of three (3) years of nursing experience in transplant, critical care or medical-surgical nursing
- Two (2) years of progressive leadership experience
- Demonstrated clinical expertise for population served with a minimum of one (1) year in specialty area
- Five (5) years of clinical experience in the transplant field
- Academic Healthcare experience
- Advanced Practice Nursing Role or Clinical Leadership experience
- Baccalaureate Degree in Nursing from an accredited School of Nursing
- Masters Degree in Nursing
- Certification in Transplantation
- Follows documented physician/licensed independent provider orders, within the boundaries of the Nurse Practice Act and established standards, policies and practice guidelines.
- Organizes and plans work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources.
- All day-to-day operations of the role are handled independently with minimal assistance.
- Advice and guidance are sought from the Manager or Director on an as-needed situational basis and when there is a major impact on activity outside the unit.
- Actions that are taken without prior approval include daily operational functions and leadership of staff.
