Registered Nurse, Home Health

Overview

Registered Nurse, Home Health at Center Well Home Health. Provide personalized, one-on-one in-home nursing care and work with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and independence at the patient’s home.

Base pay range: $70,500.00/yr - $96,900.00/yr

Sign-on bonus: $10,000

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient; provide follow-up as directed by the Clinical Manager.
• Collaborate with the physician to develop the initial plan of treatment based on orders and assessments; implement, monitor, and revise the plan with physician input as required by policy and regulation.
• Coordinate care with physical therapists, occupational therapists, home health aides, and external providers.
• Report patient care status to the patient s physician and Clinical Manager on a continuous basis.
• Execute the patient care plan with the patient and family to help achieve optimal outcomes.
• Oversee and supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; ensure documentation reflects the current plan of care.
• Discharge patients after physician and Clinical Manager consultation; complete required clinical documentation.
• Prepare medical documentation for all patients, including case conferences, medication changes, re-certifications, progress updates, and care plan updates; prepare visit/shift reports and coordinate with other health care disciplines to ensure high-quality patient care.

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred
• Strong med-surg, ICU, ER, or acute care experience
• Home Health experience is a plus
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Pay range per visit/unit: $45.00 - $63.00

Scheduled weekly hours: 40
