Registered Clinical Dietitian - RD - St. Mary s Hospital

This role supports diabetes management within specific clinical site(s) and involves collaboration with providers, diabetes self-management education/training, and quality improvement initiatives.
Responsibilities- Demonstrates knowledge of the principles of growth and development of adults; assesses data reflective of patient status and interprets information to identify patient needs by age and specific requirements.
- Manages a patient registry of adult patients with diabetes from the electronic health record.
- Establishes a collaborative, therapeutic alliance with people with diabetes (PWD) that supports shared decision making in care.
- Implements DSME/T education program in assigned site(s).
- Individually tailors diabetes self-management instruction to accommodate physiologic, psychologic, emotional, cultural and spiritual needs.
- Documents DSME/T assessment, plan and behavioral outcomes in the electronic health record; contributes data to analytics.
- Examines abnormal diabetes-related laboratory and diagnostic reports; uses clinical judgement to determine next steps and communicates proposed therapeutic options to providers both verbally and in writing.
- Participates in quality improvement activities using data from the electronic health record.
- Participates in the selection of program materials for patient use.
- Completes peer performance reviews as assigned.
- Bills for services rendered in the electronic health record and evaluates revenue integrity.
- Assumes additional tasks based on program needs.
Required Qualifications
- 4 year/Bachelor’s Degree
- Current state licensure as a Registered Dietitian
- Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)
Preferred Qualifications
- One-year of post-graduate clinical experience in a similar position preferred
- Works closely with the Clinical Support Services team.
- Other duties as assigned
