Overview
Registered Clinical Dietitian (RD) – St. Mary’s Hospital, Bon Secours Mercy Health. This role involves managing the diabetes population at specific clinical site(s), implementing diabetes self-management education/training (DSME/T), and participating in quality initiatives to improve DSME/T programs.
Responsibilities- Demonstrate knowledge of adult growth and development; assess patient data and identify individual requirements by age and needs
- Manage patient registry of adult patients with diabetes from the electronic health record
- Establish a collaborative, therapeutic alliance with people with diabetes (PWD) supporting shared decision making
- Implement DSME/T education program in assigned site(s)
- Individualize diabetes self-management instruction to accommodate physiologic, psychologic, emotional, cultural and spiritual needs
- Document DSME/T assessment, plan and behavioral outcomes in the electronic health record; contribute data to analytics
- Review abnormal diabetes-related laboratory and diagnostic reports; communicate proposed therapeutic options to providers
- Participate in quality improvement activities using data from the electronic health record
- Assist in selection of program materials for patient use
- Complete peer performance reviews as assigned
- Bill for services rendered in the electronic health record and evaluate revenue integrity
- Assume additional tasks as needed by BSPDH
- Bachelor’s degree (4-year)
- Current state licensure as a Registered Dietitian
- Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)
- One year of post-graduate clinical experience in a similar position (preferred)
- Works closely with the Clinical Support Services team
- Other duties as assigned
