Overview
The Medical Outreach Assistant serves as a liaison between the patient/family and the health systems providing navigation to facilitate access to care, resources and improved communication to assure patient follow-up, long-term chronic care management and reduce health needs.
Responsibilities- Independent action(s) required:
Sets goals and plans to accomplish role/responsibilities. Able to handle multiple priorities and manage time to meet workload goals. Documents interactions with clients in appropriate format and software. - Outreach services may be conducted by face-to-face contacts during home or community visits, telephone contact and/or written communication.
- Schedules, including requirements of shift rotations and hours of work, including weekends and holidays, may be adjusted as necessary to meet program and work requirements.
- Contact and coordinate with patients/families to facilitate access to care, resources and improved communication to assure follow-up and coordination of services.
- May work all three shifts, weekends and holidays. Individual shift rotation and schedules will be established with the Manager who will determine the needs for appropriate coverage of services.

Child, Adolescent, Adult, Geriatric.
- Experience
REQUIRED:
Prior experience working in a healthcare and/or community health setting or on-the-job training and orientation in health sciences, social services or related field. - Experience PREFERRED:
Two (2) years experience working in community service, health or social service sector.
- Education/training
REQUIRED:
High School Diploma or equivalent. - Education/training PREFERRED:
Associate or Bachelors Degree in Health Sciences or Social Services from an accredited program.
- Physical:
Lifting 20-50 lbs. Prolonged standing, Prolonged sitting, Walking (distance), Repetitive motion. - Mental/Sensory:
Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking. - Emotional:
Fast-paced environment, Steady pace, Able to handle multiple priorities, Frequent and intense customer interactions, Noisy environment, Able to adapt to frequent change.
