Community Health Worker

Overview

The Medical Outreach Assistant serves as a liaison between the patient/family and the health systems providing navigation to facilitate access to care, resources and improved communication to assure patient follow-up, long-term chronic care management and reduce health needs.

• Independent action(s) required:
  Sets goals and plans to accomplish role/responsibilities. Able to handle multiple priorities and manage time to meet workload goals. Documents interactions with clients in appropriate format and software.
• Outreach services may be conducted by face-to-face contacts during home or community visits, telephone contact and/or written communication.
• Schedules, including requirements of shift rotations and hours of work, including weekends and holidays, may be adjusted as necessary to meet program and work requirements.
• Contact and coordinate with patients/families to facilitate access to care, resources and improved communication to assure follow-up and coordination of services.
• May work all three shifts, weekends and holidays. Individual shift rotation and schedules will be established with the Manager who will determine the needs for appropriate coverage of services.

• Experience
  
  REQUIRED:
  
  Prior experience working in a healthcare and/or community health setting or on-the-job training and orientation in health sciences, social services or related field.
• Experience PREFERRED:
  Two (2) years experience working in community service, health or social service sector.

• Education/training
  
  REQUIRED:
  
  High School Diploma or equivalent.
• Education/training PREFERRED:
  Associate or Bachelors Degree in Health Sciences or Social Services from an accredited program.

• Physical:
  Lifting 20-50 lbs. Prolonged standing, Prolonged sitting, Walking (distance), Repetitive motion.
• Mental/Sensory:
  Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking.
• Emotional:
  Fast-paced environment, Steady pace, Able to handle multiple priorities, Frequent and intense customer interactions, Noisy environment, Able to adapt to frequent change.
