Purpose of the JobDeliver the highest quality care to our assisted living and memory care residents by providing oversight of their medical and emotional needs by recognizing medical issues and contacting the appropriate healthcare providers, family members, and/or responsible parties, implementing medical orders, managing and distributing medications as well as assisting with activities of daily living.
Responsibilities and Tasks- Act as shift lead for nursing assistants and med techs on a shift.
- Recognize medical issues as well as monitor the status of chronic illnesses.
- Coordinate with external health care providers such as home health and hospice.
- Administer health assessments such as LOC, UAI and ISP.
- Communicates any concerns to healthcare providers as well as family members or responsible parties.
- Knowledge of principles and processes for providing resident and personal services such as conducting needs assessments and meeting quality standards of service.
- Knowledge of principles, methods, and procedures for diagnosis and treatment of physical and mental issues with residents.
- Strong knowledge of the 5 Rights of Medication Administration-The Right:
Drug, Dose, Route, Patient, Time. - Ability to tell when something is wrong is likely to go wrong with a resident.
- Ability to listen to and comprehend information and ideas presented from residents, family members, co-workers, visitors, and medical professionals.
- Ability to communicate ideas and information to residents, family members, co-workers, visitors, and medical professionals.
- Ability to make decisions and solve problems by analyzing information available.
- Ability to perform and prioritize multiple tasks effectively and efficiently.
- Ability to work within defined time constraints.
- Ability to take initiative and follow through.
- Ability to maintain accurate records and prepare reports.
Education and Licenses
:
Requires the ability to use fractions, decimals, and commercial arithmetic. Basic knowledge of grammar, spelling, and punctuation is required. Equivalent to a 4-year high school education. Licensed Practical Nurse license required except if working on provision pending taking the state boards.
Experience
: 0 to 3 months supervisory experience. 0 to 1-year hands-on nursing experience.
