The Clinical Pharmacist ensures the overall integrity, safety, rationality, and effectiveness of the drug use control process for all patients in the assigned geographic or specialty area, including the following patient groups:
Neonates, Pediatrics, Adolescents, Adults, and Geriatrics.
- Current licensure with the Virginia State Board of Pharmacy required or obtained within the first 90 days of employment.
- Licensed as a Pharmacist with the Virginia Board of Pharmacy for continued employment.
- Experience:
Minimum of one (1) year of pharmacy practice experience in a similar setting or residency training.
- Required:
Bachelors of Science in Pharmacy from an accredited academic program. - Preferred: PGY1, ASHP-accredited residency; PGY2, ASHP-accredited residency;
Doctor of Pharmacy (Pharm.
D.) from an accredited academic program.
The monitoring, screening, and verification of all drug-related orders for assurance of proper dosage, route, dosage form, and administration schedule. Ensure expedited preparation and dispensing of all dosage forms by pharmacy personnel. Resolve drug therapy problems. Clarify and correct problematic drug orders. Designate pharmacy technicians to perform special assignments. Evaluate employees’ and students’ performance. Counsel patients regarding the safe and proper use of their medications.
Resolve equipment and operational problems. Interact with other health care practitioners in a multidisciplinary environment. Monitor the effects of medications, and recommend and/or initiate appropriate adjustments to achieve desired medication-related outcomes.
Supervise the actions of Pharmacy Interns and Certified Pharmacy Technicians as set forth in the Pharmacy Act and the Drug Control Act with related statutes and regulations of the Virginia Board of Pharmacy. Supervise the actions of pharmacy residents as set forth in Department of Pharmacy Services.
Additional Requirements- Must be available to work evenings, nights and weekends.
