Brief
Description:
As the Home Health Physical Therapist, you will provide physical therapy services in accordance with the plan of care. Provide initial and on-going assessment and evaluation of patient response to therapy. Accurately document evaluation and treatments provided.
Shift Details: PRN - paid per service. Flexible scheduling.
Requirements:
- Provide services in accordance with accepted standards of practice for physical therapy
- Document all therapy interventions
- Provide initial and ongoing assessment and evaluation of patient/family response to therapy
- Develop and revise the plan of care in consultation with the physician and other care-team members
- Provide therapeutic treatments and evaluation of equipment needs as applicable
- Advise and consult with the family and other agency personnel as applicable
- Complete documentation timely for all assessment and evaluation data, treatments, and patient’s response to therapy intervention
- Evaluate outcomes of care and complete the OASIS assessment accurately
- Provide discharge planning and evaluation
- Supervise physical therapy assistants at least every 30 days and home health aides every 14 days when appropriate
- Participate in in-service programs
- Comply with agency policies and procedures relative to infection control and safety
Required Knowledge, Skills And Ability:
- Current, unrestricted license in the Commonwealth of Virginia as a Licensed Physical Therapist
- Current CPR certification for healthcare provider
- Demonstrated ability to read, write, and speak English
- Must possess good verbal and written communication skills
- Must be able to meet deadlines, work under limited supervision
- Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization
- Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
- Proof of motor vehicle insurance
Required
Education And Experience:
- Passed an examination for physical therapists approved by the state in which physical therapy services are provided OR graduated after successful completion of a physical therapist education program approved by one of the following:
- The Commission on Accreditation in Physical Therapy Education (CAPTE)
- Successor organizations of CAPTE
- An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists
