Job Description
Coordinator assess the needs of walk-in and phone callers to ensure their referral to the service or resource to best address those needs. Coordinator assists in scheduling initial psychiatric assessment on patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders and presents symptoms to psychiatrist for best recommendation of care. Coordinator refers and triages patients to other sources of treatment when appropriate.
Coordinator answers all inquiry calls regarding treatment and the facility, presents treatment options, documents calls and sets appointments for callers seeking treatment. Coordinator works with the business office to ensure smooth transition into the admission process.
- Education:
High school degree or equivalent preferred. - Experience:
Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred. Must have strong clinical assessment skills. - License:
Valid driver's license. - Additional Requirements:
CPR certification and Crisis Prevention Training (CPI) within 30 days. May be required to work flexible hours and overtime. With proper credentials, Coordinator may be required to complete vitals check on patients.
- Provides clerical support to the Care Coordination Manager and Care Coordination Team and acts as pack up to front desk reception. Enters information into the Medik system accurately.
- Maintains appropriate stock of paperwork/forms and supplies to support the Care Coordination Department.
- Checks in patient belongings with accuracy and documents appropriately.
- Processes consent forms and coordinates admission paperwork for unit.
- Provides assistance to other units and departments as needed.
- Sends appropriate documentation to referral sources in the appropriate timeframe.
- Provides assistance with discharges.
- Schedules appointments for incoming assessments paying close attention to the needs of the patient.
- Assists in the orientation of patients to units and to outpatient programs.
