Under the Direct Supervision of the VP of Physician Services, the Patient Navigator will work in collaboration and continuous partnership with patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources to connect patients to relevant community resources, with the goal of enhancing patient health, well‑being, decreasing patient readmissions and increasing patient satisfaction.
Minimum Education- High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma/GED education will be considered.
- 3 years previous customer service experience.

- Previous experience in a customer support role. Strong phone and verbal communication skills along with active listening. Customer focus and adaptability to different personality types. Ability to multi‑task, prioritize, and manage time effectively. Ability to establish and maintain strong working relationships with staff and management; excellent organizational skills with emphasis on detail and ability to effectively communicate both verbally and in writing.
Detail oriented, team player, and an independent thinker.
