• Conduct adult/geriatric assessments that include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Coordinate with Primary Care Physicians on patient education and follow-up.
• Provide services at the member’s place of residence, including homes or Skilled Nursing Facilities.

• Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday–Saturday from 8 am–7 pm. You can work during the day, evening, and/or weekend.
• Competitive Pay – Earn income based on the number of assessments you complete. Those completing 20–100 visits per month earn $1,800–$11,500, depending on state and per‑assessment rate ($90–$115).

• Additional Licensure – Support for obtaining additional state licensure and credentialing as needed.
• Full-time Opportunities – PRN providers can transition to full‑time roles and receive a sign‑on bonus.

• Master’s Degree or equivalent experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice with prescription authority or ability to obtain it.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS, or CPR certification.
• 1 year of NP experience preferred; new graduates encouraged.
• Valid state driver’s license and adequate auto insurance coverage.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
