• Skilled Nursing Facility / Rehab

• Evaluate patients and develop individualized treatment plans

• Provide therapeutic exercises, mobility training, and functional activities

• Assess and document patient progress and adjust plans as needed

• Collaborate with interdisciplinary care teams to support patient goals

• Educate patients and caregivers on mobility, safety, and home exercises

• Maintain accurate and timely clinical documentation per facility standards
