Job Summary
NO OASIS REVIEW! We are seeking a qualified and experienced Registered Nurse Clinical Manager to lead and support the coordination of patient care services in a home health setting. This role is responsible for ensuring the quality and compliance of clinical services through oversight of patient care planning, staff coordination, and adherence to regulatory standards. The Clinical Manager plays a key role in maintaining continuity of care by reviewing referrals, assigning appropriate services, and overseeing comprehensive documentation.
This position requires availability during all operating hours and a commitment to high standards of patient care and operational excellence.
Job Type: Full-time
Pay: $92,000.00 - $ per year
Duties- Coordinate and oversee care planning for all patients under your caseload
- Review all new referrals for clinical appropriateness
- Approve and co‑sign clinical orders, including LVN and follow‑up orders
- Provide guidance on OASIS documentation and assist RNs with utilization reviews
- Facilitate case conferences, interdisciplinary communication, and care planning
- Ensure timely submission of assessments and completion of documentation in accordance with billing and compliance guidelines
- Maintain communication with physicians, schedulers, therapists, and field staff
- Support the Director of Nursing by supervising field staff as needed and assisting with scheduling
- Participate in call rotation and assist in triage responsibilities
- Monitor patient status, skilled need, and homebound status to ensure eligibility
- Coordinate corrections with the QAPI team and ensure accurate, timely documentation
- Minimum of one (1) year of recent clinical experience as a Registered Nurse
- At least one (1) year of experience in a Home Health setting is required
- Previous leadership or supervisory experience in home health or another clinical care environment is strongly preferred
- Thorough understanding of home health operations, including coordination of care, compliance with Medicare Conditions of Participation (CoPs), and management of OASIS documentation
- Proficiency with Homecare Homebase (EHRs) and experience managing patient documentation and workflows in Homecare Homebase
- Demonstrated ability to lead interdisciplinary teams, oversee patient care plans, and support clinical staff performance
- Experience with utilization review, quality assurance, and performance improvement in a home health context is a plus
- Primarily office‑based with occasional field responsibilities
- May require local travel and patient home visits as needed
- Must be able to manage multiple tasks and work in a fast‑paced environment
- Occasional exposure to varied home environments and weather conditions
- Must be able to lift up to 50 lbs and support patients as needed
