is hiring a Clinical Care Registered Nurse to join our clinic in Clinical Care RN will provide comprehensive nursing care to a diverse group of patients and families, across a lifespan in a community-based setting using the knowledge, skills, and abilities of the nursing profession. This role will have a deepened relationship with our patient population and in coordination with the Clinical Care team, you will be responsible for monitoring patient health, providing treatments, administering medications, and educating patients on health maintenance and disease prevention with the goal of improving health outcomes and facilitating access to services.
This position will work closely with medical staff to ensure quality care is provided to In collaboration with clinic leadership, the Clinical Care RN will triage, assess, plan, schedule, coordinate, evaluate, and delegate the delivery of patient-centered nursing care in the primary care setting.
Standardize tasks and workflows that support all staff and providers working to the top of their licensure. This includes patient schedule scrubbing, in-basket management and medication refills.
Work with patient to develop a plan to improve general health status.
Maintain concise, accurate and timely documentation that supports effective, efficient management of the member and that meets accreditation, contractual and legal requirements.
Other duties may be assigned as necessary in the performance of this position. Three (3) years’ experience as a RN in physician's office, family practice, or similar medical setting**.
** Has knowledge of Medicare documentation, billing and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM) and Transitional Care Management (TCM) services to include patient calls and calculation of billable time.
Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.).
Experience with Epic Electronic Medical Record.
