We are seeking a compassionate, patient-centered Nurse Practitioner (NP) or Physician Assistant (PA) to join our primary care clinic and serve as a key provider for a dedicated patient panel. This role is ideal for a clinician who values continuity of care, chronic disease management, and preventive medicine—and who is excited to enhance traditional primary care with Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) tools.
You will function as a primary care provider delivering comprehensive outpatient care to Medicare and underserved populations within a collaborative, team-based model. This is a full-time, salaried position with additional productivity-based compensation tied to RVUs, offering both stability and performance-based upside.
What Makes This Primary Care Role Unique- Deliver longitudinal, relationship-based primary care to an established patient population
- Focus on chronic disease management, preventive care, and care coordination
- Utilize RPM, CCM, and EMR-integrated technology to proactively manage complex conditions
- Practice in a mission-driven clinic prioritizing access, quality, and outcomes
- Collaborate closely with MAs, care coordinators, and clinical leadership in a team-based environment
- Active (or eligible) Washington State NP or PA license
- 2+ years of experience in primary care, family medicine, internal medicine, or geriatrics
- Strong foundation in chronic condition management and preventive care
- Comfort managing a patient panel and serving as a patient’s primary care provider
- Proficiency with EMRs, documentation workflows, and digital health tools
- Excellent communication skills with a patient-centered approach
- Competitive base salary with additional productivity-based compensation (RVUs). Total annual compensation may vary based on individual performance, patient complexity, and clinic needs, with opportunity to earn up to approximately $200,000 annually.
