Kinwell is hiring a Clinical Care Registered Nurse to join our clinic in Renton, Washington.
Kinwell’s Clinical Care RN will provide comprehensive nursing care to a diverse group of patients and families, across a lifespan in a community-based setting using the knowledge, skills, and abilities of the nursing profession. This role will have a deepened relationship with our patient population and, in coordination with the Clinical Care team, you will be responsible for monitoring patient health, providing treatments, administering medications, and educating patients on health maintenance and disease prevention with the goal of improving health outcomes and facilitating access to services.
This position will work closely with medical staff to ensure quality care is provided to Kinwell patients and will support their Clinician in managing a panel of patients.
- The Registered Nurse is an integral member of the Health Care team, overseeing and participating in patient assessment and the formulation and implementation of the care plan.
- Collaborate, communicate, and coordinate with the clinicians and other health care team members in planning and implementing patient care.
- In collaboration with clinic leadership, the Clinical Care RN will triage, assess, plan, schedule, coordinate, evaluate, and delegate the delivery of patient‑centered nursing care in the primary care setting.
- Identify at‑risk patients, develop a plan of care, and coordinate care with the patient’s involvement.
- Standardize tasks and workflows that support all staff and providers working to the top of their licensure. This includes patient schedule scrubbing, in‑basket management and medication refills.
- Use technology, such as tele‑visits and remote monitoring capabilities, to engage patients and monitor chronic health conditions.
- Administration of vaccinations and medications as appropriate. Performs testing as directed by a provider (e.g., EKG).
- Support follow up patient care gaps (e.g., colonoscopies, mammography, A1c, etc.).
- Provide Transitional Care Management (TCM) following an inpatient stay for all identified patients.
- Work with patient to develop a plan to improve general health status.
- Implement physical and psychological preventive measures.
- Promote patient self‑care responsibility for meeting their own health needs.
- Assure quality of care through development, implementation and evaluation of professional nursing standards and participation in multidisciplinary quality assurance activities.
- Maintain concise, accurate and timely documentation that supports effective, efficient management of the member and that meets accreditation, contractual and legal requirements.
- Other duties may be assigned as necessary in the performance of this position.
- Active Washington State RN licensure through the Department of Health (DOH)
- Three (3) years’ experience as an RN in a physician's office, family practice, or similar medical setting.
- BLS required, current AHA Healthcare Provider training (BCLS).
