We are currently seeking a dedicated Clinical Manager to oversee our RN case management team and ensure the highest standard of care for every patient we serve.

• Lead, mentor, and support a team of Registered Nurses (RNs) providing case-managed home health care.
• Oversee daily case management activities to ensure timely admissions, appropriate care planning, and efficient service coordination.
• Review and approve OASIS assessments and care plans to ensure clinical accuracy and regulatory compliance.
• Conduct routine chart audits and quality reviews to support documentation integrity and patient outcomes.
• Collaborate closely with physicians, therapists, home health aides, and office staff to maintain continuity of care.
• Participate in clinical onboarding, staff training, and ongoing professional development initiatives.
• Ensure team adherence to Medicare guidelines, state regulations, and agency protocols.
• Support coverage needs and manage staff productivity and performance.
• Contribute to performance improvement and patient satisfaction initiatives.

• Current RN license in NV.
• Minimum of 3 years of experience in home health nursing, with 1+ years in a supervisory or case management leadership role.
• Proficient knowledge of OASIS, Medicare Conditions of Participation, and home health best practices.
• Strong leadership, communication, and organizational skills.
• Experience with EMR systems such as Axxess is a plus.
