Key Responsibilities:

• Provide nursing visits according to the supervising RN’s plan of care.
• Maintain visit frequency and communicate necessary changes.
• Conduct home safety and environmental checks; report concerns to the RN.
• Prepare equipment and administer treatments using proper aseptic technique.
• Teach patients and families self‑care; reinforce the HHA plan of care.
• Collaborate with the RN and care team, participating in case conferences.
• Submit accurate, timely documentation each week.
• Recommend community resources to support safe home care.
• Attend staff meetings, contribute to quality improvement, and mentor new team members.

Qualifications:

• Current LPN license in Nevada (required).
• Minimum 1 year of home health nursing experience (required).
• Strong communication and teamwork skills.
• Ability to work independently and manage a flexible schedule.
