Full-Time & Part-Time RN Case Manager – Home Health Service Area:
Reno, Sparks

We are looking for an experienced Registered Nurse (RN) in the Reno/Sparks area to join our growing team. If you’re ready to work for an agency where your voice matters and your work truly makes a difference, we’d love to meet you.

Responsibilities

Provide skilled, one-on-one nursing care in patients’ homes.

Complete thorough OASIS assessments and design individualized care plans.

Monitor progress, document accurately, and coordinate with the interdisciplinary team.

Educate patients and families on care, medications, and self-management strategies.

Collaborate with physicians, therapists, and aides for seamless care delivery.

Participate in case conferences and quality improvement initiatives.

What We’re Looking For

Active RN license in Nevada

Minimum 4 years total nursing experience

At least 2 years in a Medicare-certified home health agency

Proficient in OASIS assessments and Medicare documentation

Strong clinical judgment, communication, and organizational skills

Reliable transportation with valid driver’s license and current auto insurance

Familiarity with EMR systems like Axxess (preferred)
