As an Occupational Therapist
, you’ll provide personalized therapy in a home setting
, helping patients regain independence in daily living activities through customized treatment plans and adaptive strategies
.

• Develop personalized treatment plans focused on ADLs, motor skills, cognitive function, and environmental modifications
  .
• Conduct comprehensive patient assessments and establish realistic therapy goals
  .
• Provide hands‑on occupational therapy services
  , helping patients increase mobility and functionality
  .
• Educate patients and caregivers on adaptive techniques, energy conservation, and safety strategies
  .
• Collaborate with nurses, physical therapists, and other healthcare professionals to provide comprehensive care
  .
• Maintain accurate and timely documentation following regulatory standards
  .
• Stay current on home health regulations and evidence‑based therapy techniques
  .

• Valid State License as an Occupational Therapist
• Minimum 1 Year of Experience in home health or acute care
• Strong clinical skills with a patient‑first mindset
• CPR Certification (Required)
• Excellent communication & interpersonal abilities
• Reliable transportation for home visits
• Understanding of Medicare home health regulations & documentationli>
