Job SummaryWe are seeking an experienced and dedicated full‑time & part‑time Speech Therapist to join our home health team, specializing in providing comprehensive speech and language therapy services to patients in the comfort of their homes. The ideal candidate should have a strong background in home health care, with expertise in assessing and treating communication and swallowing disorders.

Responsibilities- Conduct thorough assessments of patients’ speech, language, cognition, and swallowing abilities, identifying areas of impairment and establishing realistic therapeutic goals.
- Develop and implement individualized treatment plans based on assessment findings, incorporating evidence‑based practices to enhance communication and swallowing function.
- Provide skilled speech therapy services to patients in accordance with physician orders, addressing issues such as articulation, voice, fluency, language comprehension, and swallowing disorders.
- Collaborate with interdisciplinary team members, including nurses, occupational therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
- Educate patients and their families on speech and language exercises, strategies for communication enhancement, and safe swallowing techniques.
- Instruct and guide patients through prescribed therapeutic exercises and activities to improve speech and swallowing function.
- Document patient progress, treatment interventions, and any changes in the patient’s condition accurately and timely in accordance with regulatory standards.
- Participate in case conferences and team meetings to discuss patient care plans and contribute to overall care coordination.
- Stay updated on current speech therapy practices, emerging trends, and home health regulations to provide evidence‑based and up‑to‑date care.
- Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment.
- Valid state license as a Speech Therapist.
- Minimum of 1 year of experience in home health speech therapy and/or an acute care setting.
- Proficient clinical skills with a focus on patient‑centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Understanding of Medicare home health regulations and documentation requirements.
