As a Per‑Diem Occupational Therapist, you’ll provide personalized therapy in a home setting, helping patients regain independence in daily living activities through customized treatment plans and adaptive strategies.

• Conduct comprehensive patient assessments and establish realistic therapy goals.
• Develop personalized treatment plans focused on ADLs, motor skills, cognitive function, and environmental modifications.
• Provide hands‑on occupational therapy services, helping patients increase mobility and functionality.
• Educate patients and caregivers on adaptive techniques, energy conservation, and safety strategies.
• Collaborate with nurses, physical therapists, and other healthcare professionals to provide comprehensive care.
• Maintain accurate and timely documentation following regulatory standards.
• Stay current on home health regulations and evidence‑based therapy techniques.

• Valid State License as an Occupational Therapist.
• Minimum 1 year of experience in home health or acute care.
• Strong clinical skills with a patient‑first mindset.
• CPR Certification (required).
• Excellent communication & interpersonal abilities.
• Reliable transportation for home visits.
• Understanding of Medicare home health regulations & documentation.
