Job Summary:

We are seeking an experienced and dedicated Occupational Therapist to join our compassionate home health team. The ideal candidate should possess a strong background in home health care, with proficiency in evaluating and implementing effective occupational therapy interventions tailored to enhance the independence and well-being of patients in their homes.

Responsibilities:

Conduct comprehensive assessments of patients' occupational performance, identifying areas of impairment and formulating realistic therapeutic goals.

Develop individualized treatment plans based on assessment findings, focusing on activities of daily living (ADLs), adaptive equipment, and environmental modifications to improve patient functionality.

Provide skilled occupational therapy services to patients in accordance with physician orders, addressing issues such as fine motor skills, cognitive function, and sensory integration.

Collaborate with interdisciplinary team members, including nurses, physical therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.

Educate patients and their families on adaptive techniques, energy conservation, and strategies to enhance independence in daily activities.

Instruct and supervise patients in prescribed therapeutic exercises and activities to improve motor coordination and overall functional abilities.

Document patient progress, treatment interventions, and changes in the patient's condition accurately and timely in accordance with regulatory standards.

Participate in case conferences and team meetings to discuss patient care plans and contribute to the overall care coordination.

Stay abreast of current occupational therapy practices, emerging trends, and home health regulations to provide evidence-based and up-to-date care.

Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment.

Qualifications:

Valid state license as an Occupational Therapist.

Minimum of 1 years of experience in home health occupational therapy and/or in an acute care setting.

Proficient clinical skills with a focus on patient-centered care.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a team.

Understanding of Medicare home health regulations and documentation requirements.

Current CPR certification.

Reliable transportation for home visits.
