- Collaborate with other staff to facilitate meaningful, high quality community connections, personal development and recreation opportunities
- Encourage and support individual independence, choice, and self-expression in the areas of self care, communication, recreation, socialization and participation
- Act as a role model while encouraging and assisting the individual to participate in the daily activities and responsibilities of their personal life and their home
- Support residents to participate and engage in the community through leisure activities, and fostering social networks that align with the individual's interests and person centered plan
- Establish and maintain rapport with residents and maintain positive relationships with key stakeholders
- Keep accurate records and complete necessary documentation requirements, including but not limited to Person Centered Plan updates, daily logs and incident reports
- Post secondary education and/or experience in Human Services and/or disability support with preference given to individuals with a background in Person Centered concepts
- A positive attitude and belief that persons with disabilities have the right and responsibility to define and pursue their life choices and pursue active participation and involvement in the community
- Preference to individuals that have certification in PART, Food Safety, TLR, Medication Administration
- A satisfactory Criminal Record Check including Vulnerable Sector Search
- Must have a valid Class 5 Saskatchewan Driver's License and use of a reliable vehicle, including $2 million liability insurance on the vehicle you will bring to work each day
- The ability to work 12 hour shifts
- Have open availability for each day of the week to include Saturdays and Sundays
Please note:
Supports are provided 24 hours a day, 7 days per week. Shift work is required to include both Saturdays and Sundays.
