Overview

The County of San Mateo Family Health Services Division is seeking an experienced and qualified Senior Community Worker - Spanish Speaking Preferred to serve in the Enhanced Care Management (ECM) Program within California Children's Services (CCS), Family Health Services Division. To be eligible for ECM, one must have CCS and have additional care management needs beyond the CCS condition. CCS is a tax-supported statewide program that helps children and young adults up to 21 years old who have certain medical conditions.

CCS provides services such as arranging, directing, and paying for medical care, occupational and physical therapy, equipment, and rehabilitation.

Responsibilities
  • The Senior Community Worker provides enhanced care management to the highest-need members and intensive coordination of health and health-related services to designated CCS clients through in-person field/home visits and electronic contacts via telephone or video calls.
  • Provide core services such as outreach and engagement, comprehensive assessment and care management planning, coordination of and referral to community and social support services, enhanced coordination of care, member and family support, health promotion, and comprehensive transitional care.
  • Work under the direction of the Community Program Supervisor and collaborate with the Medical Therapy Unit and Health Plan of San Mateo Whole Child Model to ensure client needs are met.
  • Navigate electronic health records and provide accurate and descriptive documentation in client files.
  • Duties may include additional related responsibilities as assigned.
Qualifications
  • The ideal candidate brings at least two years of recent experience conducting comprehensive assessments, care coordination, health advocacy, and referrals to community-based services.
  • Familiarity with the CCS and ECM programs is highly desirable.
  • Ability to work independently and collaboratively within a multidisciplinary team; clear verbal and written communication; flexibility in adapting to changing environments.
  • Possession of a valid California Driver’s License or equivalent is required.

Duties
  • Assess and monitor client needs and develop individual care/recovery/support plans based on client needs.
  • Prepare and/or coordinate informational and outreach materials, basic correspondence, reports, and other documents regarding program activities.
  • Assist staff in developing strategy to engage with the community; interpret community needs and cultural patterns to professional staff members.
  • Provide crisis intervention and arrange for special services to assist clients with serious personal, financial, employment, and/or family issues.
  • Establish and maintain collaborative relationships with community resources.
  • Conduct home and field visits to assess and monitor client needs and ensure adherence to care plans.
  • Provide information and refer clients to appropriate social services agencies; research community resources; coordinate outreach and referrals with County staff.
  • Provide educational services; facilitate educational programs and activities to support client health/wellness, recovery, self-sufficiency, or employment goals.
  • Conduct outreach to inform individuals about programs and services and promote participation in activities.
  • Identify home, family, or community conditions needing improvement and develop measures to address them.
  • Follow up and monitor cases; develop and maintain individual development plans with clients and the management team.
  • Monitor action plans and contact clients to assess progress and identify needed support for interim goals.
  • Advocate with external agencies, employers, and organizations to ensure client representation and information, and to resolve issues with third parties.
  • Assist clients in daily living activities, such as completing applications, transportation, budgeting, and navigating support systems.
