We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full‑Time or Part‑Time Physical Therapist Assistant (PTA) in Redwood City, CA.

Position OverviewWe are seeking an experienced, self‑driven PTA to join our dedicated team and provide exceptional care throughout the region. You will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

- Provide physical therapy services within the scope of care, following the patient’s individualized plan and in coordination with the interdisciplinary team.
- Administer treatments, monitor patient conditions, and report any changes in patient condition to the Physician, Case Manager, and/or Director of Nursing as needed.
- Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
- Comply with all company policies and procedures to ensure high standards of care.
- Current Physical Therapist Assistant (PTA) license to practice in California – required.
- At least 1 year of experience in home health or hospice care – required.
- Current CA driver’s license.
- Able to travel within a 15‑25 mile radius on a regular basis.
